Transition is the process of developing the skills needed for independent and successful postsecondary life – in whatever path an individual chooses to take. NESCA provides vision-driven, person-centered transition services that support students and young adults as they prepare for life after high school and beyond.

NESCA offers a range of transition assessment services designed to identify and support a student’s postsecondary living, social, educational, and vocational goals, as well as their related strengths and needs. Families are encouraged to begin with a one-hour parent intake and consultation prior to starting the assessment and planning processes.

NESCA’s transition assessment services are designed to support students, families, schools, and professionals and are provided by our team of Transition Specialists, Special Educators, Occupational Therapists, and related providers.

What We Offer:

  • Transition Planning

Help individuals identify and develop meaningful postsecondary goals and plans

Collaborate with families, schools, and community providers to identify supports, services, and strategies needed for individuals to thrive

Participate in the IEP process to help align transition services with student goals and their long-term vision

  • Transition Consultation

Short-term consultation to address specific transition questions, including college readiness, summer planning, and appropriate programs or supports

Coordination or case management services to help build a team of providers who deliver services related to an individual’s transition plan

Program consultation and case coordination for schools and families seeking to refine services, develop curricula, build provider teams, or implement postsecondary transition programs

Comprehensive transition assessments to clarify goals and evaluate interests, strengths, needs, and readiness for future educational, vocational, social, and independent living demands in preparation for life after high school

Functional observations across real-world settings, including schools, college campuses, workplaces, volunteer sites, recreational settings, and community environments

Consultation with schools to strengthen transition assessment practices and identify effective tools and strategies

  • Training and Professional Development

NESCA also provides transition-related training and consultation for schools, professionals, and organizations.
